The recruitment target was exceeded in this trial due to the enormous enthusiasm of our trial centres and these are listed below with the nominated surgeon, radiologist and coordinator for each hospital. We also thank all the other clinicians within the named hospitals and from local supporting hospitals, who were responsible for the referral of almost one third of the patients to the trial centres.
